Dog Adoption Form

Please list all pets you have owned. For current animals please include type of animal, breed, sex, age and if they are spayed or neutered. For animals you no longer have please explain why you no longer have them and provide details. (If they were euthanized, rehomed privately, taken to a shelter, returned to a shelter, died etc)

CAARS is required to see a lease or rental agreement showing permission to have animals in your home. If no lease or rental agreement is available please have written permission from your landlord. There is a file upload button below this form.

I have read the above information carefully and have filled out this application honestly. I understand that omission of information and/or failure to answer all the questions can result in this application being declined.

Any personal information collected is only used by Camrose Animal Alliance Rescue Society for the purpose defined at the time of the collection or a use that complies with these purposes. We do not share your information with any third parties.

POLICIES & PROCEDURES

CAARS looks to find great matches for both our pets and potential adopters and may sometimes suggest a different dog or puppy that may be more suitable to your situation.

*Please note that the animals in our care have uncertain pasts. We do our best to guess at age/breed/size, but we can never be certain. We also cannot guarantee health or temperament.

RETURN POLICY

CAARS accepts returns in most instances, please contact us to arrange. Please note sometimes placement of a returning animal can take up to a couple of weeks to accomodate, depending on available foster space and the individual needs of the returning animal.

VACCINATIONS/DEWORMING

CAARS provides vaccinations for all animals due while in our care. The adoptive family is responsible for vaccinations and continued deworming that come due after the adoption is completed, which can include on-going treatment for minor ailments such as worms.

SPAY/NEUTER POLICY

All of our animals are spayed or neutered prior to adoption; this cost is included in our adoption fee.

IF YOU HAVE DOGS AND/OR CATS

We strongly suggest slow introductions if you choose to welcome a new animal coming into your home.

STEP 01. ADOPTION APPLICATION

To adopt, first complete our adoption application form.

STEP 02. APPLICATION REVIEW

If your application is accepted we will contact you and a deposit of $250 is then due to hold that animal for you. This can be etransferred to camroseanimalalliance@gmail.com

STEP 03. SPAY/NEUTER SURGERY IS BOOKED

Your new pet will be ready to go home 24 hours after surgery and upon payment of the balance of the adoption fee. Please be prepared for your new pet, with food, a bed, toys, and bowls, before he/she comes home.
